HOW TO USE THE ONLINE CALENDAR TO SCHEDULE AN APPOINTMENT

  1. Select the button that best describes you from the options listed.

  2. Select the appointment type you’d like to schedule.

  3. View available dates and times and select your appointment time (availability shown is completely up to date)

  4. Provide your name, email address, phone number and carefully read and complete consent forms.

  5. Pay for your appointment in advance or reserve your appointment by adding a valid credit card on file.

  6. You’ll receive immediate email confirmation of your appointment, along with a text & email reminder 24 hours before your appointment.

  7. Don’t see the date and time you’d like available on the calendar? Select “waitlist” to be notified if it becomes available.
